To Submit a Referral for Counseling & CANS 3.0

For counseling & CANS 3.0 assessments please complete our online referral tool. Completing the referral on our website is the fastest way to get the process started--even if you're not sure we have immediate availability in your area.

We specialize in in-home trauma-informed mental health counseling for children ages 3 and up. We are also able to meet with the child/family wherever is clinically appropriate (i.e. daycare, school, telehealth).

As soon as your request is submitted, we will verify insurance and get to work assigning a local counselor right away! Our Administrative Coordinator will reach out to you to introduce the assigned counselor or discuss options as soon as possible. For any questions on if a referral was received, or to check status of existing referral requests, please email [email protected].

*Anyone on the treatment team can submit the referral (Caregiver, DFPS Caseworker, Placement Agency Case Manager, CASA, Insurance, etc.). Thank you for your referral! We ask that you please coordinate with the rest of the team to avoid duplicate referrals and ensure services are needed.

*If you'd like to work with a specific counselor, please type their name in all caps in the "Treatment Preferences" section on the referral to ensure we assign the correct person! If you miss this step, services will be delayed.

*We are in network with Superior HealthPlan, Traditional Medicaid, Aetna Better Health-Medicaid, Cooks, Parkland, and Traditional Medicare.

If you are currently working with one of our counselors and have questions about treatment goals, session frequency, etc., please contact the counselor directly. 

Every time a counselor is assigned, we send a Counseling Referral Confirmation introducing the assigned counselor with all of their contact information.  Speaking directly with the counselor is typically the best way to communicate about an ongoing case.

How to request healthcare records

You may request copies of your counseling records at any time. To request records:

  • Submit a written request by email to [email protected]
  • Please include the patient's full name, date of birth, the records you are requesting, and the email address where you’d like them sent.

We will acknowledge and process your request as required by Texas state law

How to file a consumer complaint

"If you believe your rights have been violated or you have concerns about professional conduct, you may email [email protected] and file a complaint with the Texas Behavioral Health Executive Council.

Complaints must be submitted in writing. BHEC reviews all complaints and will notify you regarding next steps
